The Cochrane Infectious Diseases Group have produced checklists to plan the dissemination of their reviews, and these are available to any Cochrane Review Groups to adapt and use for their own dissemination planning. 

This checklist will help you create a plan a strategy to disseminate your review, by tracking your target audience, the channels you will use to reach them and the timelines that your plan will encompass.
